USCIS Immigration Exams (I-693)

Everyone who applies for a green card (adjustment of status) in the United States needs to have a medical examination performed by a doctor who is authorized by U.S. Citizenship and Immigration Services (USCIS).

During the Exam

The medical exam will include a review of your medical history and a physical exam. The physical examination will assess your overall physical condition including:

  • Vital Signs (blood pressure, breathing rate, pulse rate, temperature, height, and weight).
  • Vision
  • Motor Functions
  • Updated Health History

The doctor will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea. After the examination, your doctor will complete Form I-693 and seal it in an envelope for you to submit with USCIS. You will be given a copy of the completed form before sealing the primary Form I-693 to prevent any alterations. USCIS does not accept forms in envelopes that have been opened or altered.

Department of Transportation (DOT) Exams

The Department of Transportation (DOT) requires that all employees who operate commercial vehicles have a medical examination. This DOT physical exam is completed by physicians at Poipu MD and certified by the Federal Motor Carrier Safety Administration.

A DOT physical exam is valid for up to 24 months. If the medical examiner deems it necessary to monitor a condition such as high blood pressure, then the certification may be valid for less than 24 months.

During the Exam

A DOT physical exam is a comprehensive evaluation that ensures there are no physical conditions that would prevent the person from operating commercial vehicles safely. As such, patients will be tested for the following:

  • Vision Test
  • Hearing Test
  • Urinalysis
  • Blood Pressure/Pulse Rate
  • Physical Exam

Please bring any documents listing medication that was administered by a doctor in the month with you. In the case of a medical disability, please supply a medical release form to illustrate the doctor’s approval for you to work with this condition.

Children’s Sports Physicals

Poipu MD physicians are qualified to perform sports physicals for kids and teens. The sports physical is given to determine if a child or teen’s body is healthy and capable of participating in certain sporting activities. Some schools require a sports physical for children before they can join a school sports team or club.

During the Exam

A health care provider will review your child’s medical history, any pre-existing conditions, allergies, medications, and previous injuries. A general screening will also be conducted that includes:

  • Vision Test
  • Blood Pressure/Pulse Rate
  • Check the child’s heart, lungs, ears, nose, and throat
  • Evaluate their posture, joints, strength, and flexibility

At the end of the exam, your physician will explain any findings and discuss the next steps with you. They may recommend vaccinations, depending on the child’s age. Parents are encouraged to bring immunization records to the appointment.

School Physicals

For children heading back to school, a proper annual physical or checkup is important to evaluate any issues your growing child may be dealing with. The Hawaii Department of Education requires that all students entering school in Hawaii have a completed Student’s Health Record. We can complete this document for you.

The medical provider will conduct a physical exam to check for any injuries or illnesses. A doctor will also use this time to review the child’s overall health status, including their nutritional habits, exercise patterns, and academic performance.

During the Exam

Back to school physical exams will vary based on the child’s age, but typically the provider will:

  • Record your child’s height, weight, blood pressure, pulse rate, and BMI
  • Check your child’s heart, lungs, lymph nodes, abdomen, skin, eyes, ears, nose, and throat
  • Evaluate your child’s vision, joints, muscles, and spine
  • Discuss the medical history of your child and your family

Please bring your child’s immunization records and a complete list of medications (prescription and over-the-counter medications).

Scuba Medical Evaluation

The scuba medical evaluation may be required prior to the issuance of a certification card or in order to participate in a scuba dive with a commercial tour company. This includes an exam and lung function analysis, which allows Poipu MD doctors to determine your ability to scuba dive safely.

During the Exam

Before you begin, please fill out any necessary forms provided by Poipu MD. During your scuba medical exam, you may receive the following tests:

  • Vision Test
  • Hearing Test
  • Heart Disease Screening
  • Urine Analysis
  • Lung Test (Spirometry)
  • Fitness Test

Please bring any previous medical records, logbooks, and diving qualifications with you.
